First request the mailer on the sponsor's site (it's free), print it, then mail it as below.

Each web form entry must be manually keystroked and individually submitted by the Entrant personally. Each eligible mailer will receive eighteen (18) Entries. To be eligible, mailers must be postmarked during the Sweepstakes Entry Period and must be received by Sponsor within five (5) business days thereafter. Each mail-in entry request form must be: hand-addressed by the Entrant personally, folded and taped per the instructions and mailed individually with postage-paid. Bulk mailings, multiple requests enclosed in a single envelope, and mailings bearing metered or pre-printed postage indicative of bulk processing are prohibited and will not be processed or entered into the Sweepstakes.
