Box 6 Newark, NY 14513 ATTN: [Title of Applicable Sweepstakes] Limit one (1) entry request per separately mailed outer envelope. To be eligible, each envelope must be postmarked during the applicable Sweepstakes Entry Period and received by Sponsor within two (2) business days after the end of that period.
